Register as a patient

All new patients be required to complete a registration form, questionnaire and attend for a new patient interview. This will help us to get to know your past medical history.  You can download the form here or pick up a copy from our reception. Please fill out the form and return it to our reception.

All new patients will be asked to provide two forms of identification.  One must contain photographic evidence and the other must confirm your address.  Examples include: passport, driving licence, utility bill, bank statement etc.

Online patient access

We offer a range of services to patients using the online portal.

They include:

  • Make/amend/cancel an appointment
  • Change your contact details
  • View your medical record
  • Repeat prescription request

Please note:  Due to the corona virus, we are currently undertaking telephone consultations only.

If the Doctor deems that it necessary to see you, they will either arrange a face-to-face appointment on the day, or a video consultations can be undertaken.

If your condition is non-urgent, you can expect to see a GP within two working days. You may have to wait longer if you want to see particular GP or wish to book an appointment for a particular day or time slot. If you don’t need an appointment within two working days, you also have the option to book up to eight weeks in advance if this is more convenient for you. Nurses based in our practice treat patients for a wide range of common conditions. You can expect to see a nurse within one working day.

Over 75s

You will be offered a yearly health assessment either at the surgery or in your own home (housebound patients only). Please contact the surgery to book an appointment.
