Register as a patient

All new patients be required to complete a registration form, questionnaire and attend for a new patient interview. This will help us to get to know your past medical history.  You can download the form, for patients over 18 years old, click  here or pick up a copy from our reception. Please fill out the form, print it off and return it to our reception.

If you wish to register your child at the practice, you will also need to complete a Registration Form for Under 18’s.  Click here to access the application form.

New Patients

Supplementary Questions

Anybody in England can register with a GP practice and receive free medical care from that practice.
However, if you are not ‘ordinarily resident’ in the UK you may have to pay for NHS treatment outside of the GP practice. Being ordinarily resident broadly means living lawfully in the UK on a properly settled basis for the time being. In most cases, nationals of countries outside the European Economic Area must also have the status of ‘indefinite leave to remain’ in the UK.

These questions are optional and your answers will not affect your entitlement to register services at Heath Street Health Centre. Click here to complete the supplementary questions.

Over 75s

You will be offered a 6 month or 12 month health assessment either at the surgery or in your own home, (housebound patients only). Please contact the surgery to book an appointment.
