Register as a new patient

 

Please complete all the relevant fields and we will contact once your registration has been approved to attend an appointment for your new patient health check.

All new patients will also be required to complete a health questionnaire. This will help us to get to know your past medical history.

You can also pick up a copy of our registration form from our reception.

Alternatively, you can print it off and return it to the practice.

After you have completed and submitted the form you must attend the surgery within 7 days with two forms of identification.

Acceptable identification includes:

  • Passport

  • Birth Certificate

  • Tenancy Agreement

  • Driving Licence

Supplementary Questions

Anybody in England can register with a GP practice and receive free medical care from that practice.

However, if you are not ‘ordinarily resident’ in the UK you may have to pay for NHS treatment outside of the GP practice. Being ordinarily resident broadly means living lawfully in the UK on a properly settled basis for the time being. In most cases, nationals of countries outside the European Economic Area must also have the status of ‘indefinite leave to remain’ in the UK.

Once your registration is complete you will be able to book appointments, order repeat prescriptions and view your medical records.  Click here to register.

  • Access to NHS 111

Summary Care Records (SCR)

Your SCR is created automatically when registered with a GP practice in England.

You can talk to us at the practice about opting out of including Additional Information about long term conditions, care preferences or specific communication needs.

Over 75s

You will be offered a 6 month or 12 month health assessment either at the surgery or in your own home, (housebound patients only). Please contact the surgery to book an appointment.
