You can now register as new patient by clicking here or by scanning the barcode below.
Please complete all the relevant fields and we will contact once your registration has been approved to attend an appointment for your new patient health check.
All new patients be required to complete a registration form, questionnaire and attend for a new patient interview. This will help us to get to know your past medical history.
You can also pick up a copy of the registration form from our reception. Please complete the form and click submit. Alternatively, you can print it off and return it to our reception.

Once your registration has been approved, we will send you an invitation for a health check. Click the link below to find what is involved.
Supplementary Questions
Anybody in England can register with a GP practice and receive free medical care from that practice.
However, if you are not ‘ordinarily resident’ in the UK you may have to pay for NHS treatment outside of the GP practice. Being ordinarily resident broadly means living lawfully in the UK on a properly settled basis for the time being. In most cases, nationals of countries outside the European Economic Area must also have the status of ‘indefinite leave to remain’ in the UK.
These questions are optional and your answers will not affect your entitlement to register services at Heath Street Health Centre. Click here to complete the supplementary questions.
Online patient access
If you have online access, you can register with the Patient Access App.
Once your registration is complete you will be able to book appointments, order repeat prescriptions and view your medical records.

Over 75s
You will be offered a 6 month or 12 month health assessment either at the surgery or in your own home, (housebound patients only). Please contact the surgery to book an appointment.
